Location: Citrus Heights (formerly Allegiant of Mesa), Mesa, AZ
Salary: $42.00–$52.00/hour
Job Type: Full-Time, 8-hour shifts, Monday–Friday (flexible hours)
About Us
Join Citrus Heights, a 202-bed Skilled Nursing Facility (SNF) and Long-Term Care (LTC) center, recently acquired by Bandera Therapy, an Ensign affiliate. Specializing in ventilator and airway care, we’re looking for a passionate Physical Therapist to join our in-house therapy team in Mesa, AZ, and make a meaningful impact on patient lives.
What You’ll Do

Conduct patient screenings and evaluations to assess needs

Create and implement personalized, evidence-based care plans

Monitor and adjust treatments to optimize outcomes

Collaborate with families and healthcare teams for holistic care
