American Traveler is hiring a travel RN Home Health Case Manager in the South Chicagoland area, requiring at least 1 year of home health experience and Epic proficiency.

Home health setting covering the South Chicagoland Patient Service Area (PSA), with rare overflow visits to select additional zip codes (60617, 60619, 60620, 60643, 60628)

Patient population includes chronic disease management (CHF, COPD, DM), Foley catheters, wound care, PICC/infusion therapy, and LVAD
-
Typical caseload is 6 visits per day, adjusted when OASIS visits are included
-
Schedule is Monday–Friday, 8-hour day shifts (40 hrs/week), beginning at 0800 until the last patient visit is complete
-
Every 3rd weekend rotation required; weekend shifts can be used to make up holidays in lieu of unpaid time off

EMR: Epic Remote Client (any Epic experience accepted)

IV pump: Sapphire

Active Illinois RN license required

Current AHA BLS certification required

Valid driver's license, personal vehicle, and auto insurance required

Minimum 2 years of RN experience, with at least 1 year in home health

Most recent experience must be within the last year

OASIS assessment experience required

Proficiency in Epic EMR required

Skills required include wound care/management, Foley catheter care, PICC/infusion therapy, central/PICC line dressing changes, and de-accessing ports

Comfortable working with an interdisciplinary team

At least 1 professional reference dated within the past year, from a Supervisor, Manager, Director, or Charge RN

Candidate DOB and last 4 digits of SSN required for consideration

Field-based role starting from home each day, traveling to patients' homes throughout the assigned service area

Daily 8:00 AM remote morning huddles conducted via Microsoft Teams

In-person team meetings held at a central location every other month

Support includes a manager/supervisor available by phone and a Home Health Aide available as needed (not during RN visits); a scheduler will assign new patients while the RN manages scheduling for their own active caseload

Orientation includes up to 2 weeks of unit onboarding: Epic setup followed by a preceptor week, with additional time available as needed for competency

RTO must be approved by the manager; maximum 7 days allowed during the contract

Scrub color: any solid color, professional attire — no graphics, logos, or designs

Local candidates are accepted at the same rate; no radius restrictions apply
