Responsibilities
- Provide personal care assistance including bathing, grooming, dressing, and mobility support while respecting client dignity and privacy.
- Assist with activities of daily living (ADLs) such as meal preparation, medication reminders, and light housekeeping to maintain a safe and comfortable living environment.
- Monitor patients’ health status by observation, tracking changes, and communicating updates to healthcare teams or family members as appropriate.
- Support clients with mobility using proper lifting techniques.
- Assist with medication reminders following prescribed care plans while adhering to HIPAA regulations and confidentiality standards.
- Offer companionship and emotional support, engaging clients in social activities or hobbies to promote mental well-being.
- Maintain accurate documentation of care provided, patient observations, and any incidents in compliance with home health or hospice care protocols..
Qualifications
- Prior experience working with individuals with disabilities, dementia, Alzheimer’s disease, or in senior care settings such as assisted living or nursing homes.
- Knowledge of patient observation techniques, medication management.
- Familiarity with care plans development and implementation tailored to individual needs across diverse settings including home & community care environments.
- Strong communication skills for effective interaction with clients, families, healthcare providers, and team members.
- Demonstrated understanding of HIPAA regulations related to patient privacy and confidentiality.
- Valid driver’s license and reliable transportation
