Title: Home Care Coordinator LVN

Shift: 8am – 4:30pm, Mon – Fri. The role would be responsible for completing visits to participants in the field, so there would need to be some flexibility to start earlier or stay later.
Hourly Comp: $38/hr - $43/hr
Position Summary: This position offers an opportunity for a mission-driven team player with experience managing home care services to develop and implement home care support for PACE program participants, including the coordination of all durable medical equipment and community-based care. The nurse serves as a collaborative member of the Interdisciplinary Team (IDT), helping to create care plans that support participants in achieving their goals. The role involves traveling to participants’ homes to conduct assessments, monitor care delivery, and ensure quality and safety. When not in the field, the nurse works from the PACE Center to complete documentation, coordinate care with the IDT, and perform administrative responsibilities. This position is well suited for a compassionate and organized nurse who values both independent fieldwork and collaborative, team-based coordination.
Responsibilities:
- Coordinates home care services for participants through contracted home care agencies and ensures that services are delivered in alignment with each participant’s individualized plan of care.
- Participates as an active member of the Interdisciplinary Team (IDT), completing home assessments, reassessments, change-of-condition evaluations, care plans, and collecting information related to home care services.
- Provides nursing care within the LVN scope of practice, delivering high-quality, person-centered care to participants.
- Educates participants and caregivers on in-home care, safety practices, and self management to promote independence and well-being.
- Responds promptly and professionally to participant and family inquiries or concerns regarding home care services.
Requirements / Certifications:
- Passion and mission orientation for serving high-risk seniors and frail older adults.
- Demonstrates compassion, patience, and respect when working with older adults and their families.
- Current and active Licensed Vocational Nurse (LVN) license in the State of California.
- CPR and First Aid certification required within 60 days of employment.
