Sign-on bonus!!! We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to serve as a Charge Nurse in our healthcare facility. The ideal candidate will possess extensive clinical experience, including critical care, long-term care, and acute care settings. As a Charge Nurse, you will oversee patient care activities, coordinate nursing staff, and ensure compliance with healthcare standards. This role requires strong leadership abilities, proficiency with electronic health records (EHR/Epic), and a deep commitment to delivering exceptional patient care across diverse medical environments such as hospitals, nursing homes, assisted living, and home care. The successful candidate will demonstrate expertise in medical documentation, medication administration, IV infusion, and specialized care such as dementia, Alzheimer's, and pain management.
Responsibilities
- Supervise and coordinate nursing staff to ensure high-quality patient care
- Conduct comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations
- Administer medications, injections, IV infusions, and perform catheterizations with precision and adherence to safety protocols
- Document all patient interactions accurately using EMR/EHR system MatrixCare
- Oversee discharge planning and collaborate with multidisciplinary teams for seamless patient transitions
- Ensure compliance with HIPAA regulations and infection control standards
- Provide education on health coaching, medication management, nutrition, and chronic disease management including dementia and memory care
- Assist in diagnostic evaluations such as laboratory specimen collection/processing and medical imaging coordination
- Lead emergency response efforts including airway management and ventilator support when necessary
- Support patients with disabilities or developmental challenges through specialized care plans in settings like assisted living or home & community care
Qualifications
- Valid Licensed Practical Nurse (LPN) license in good standing
- Proven experience in critical care environments such as ICU or Emergency Room; experience in Level I & II Trauma Centers preferred
- Familiarity with EMR/EHR systems including Epic, Cerner, Athenahealth; proficiency in ICD-9/ICD-10 coding is advantageous
- Extensive knowledge of medical terminology, anatomy & physiology, pharmacology, and diagnostic procedures
- Experience administering injections, phlebotomy, catheterization, tube feeding, suturing, and sterile techniques
- Strong assessment skills for patient evaluation across pediatric to geriatric populations including neonatal ventilator management and hospice care experience
- Ability to work effectively with diverse patient populations including those with dementia or behavioral health needs
- Excellent communication skills for collaboration with healthcare teams and family members
- Commitment to maintaining HIPAA compliance and infection control protocols
