Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, mobility, and toileting to ensure comfort and dignity
- Medication reminders, and reporting changes to healthcare professionals.
- Support patients with memory care needs such as Alzheimer’s disease or dementia by implementing personalized care plans and providing a calming environment.
- Facilitate safe patient transfers using manual handling techniques and equipment like Hoyer lifts or patient transfer methods.
- Prepare nutritious meals aligned with dietary restrictions and assist with meal consumption to promote proper nutrition.
- Maintain a clean and safe environment through light cleaning duties and adherence to infection control protocols.
- Attend scheduled appointments or community outings, and ensuring safety during travel.
- Document care activities accurately while respecting patient privacy and confidentiality in compliance with HIPAA regulations.
- Support patients with neurodevelopmental or developmental disabilities by implementing tailored support strategies and engaging in meaningful social interactions.
Skills
- Strong patient observation skills to detect changes in condition or behavior promptly.
- Knowledge of dementia care, Alzheimer’s care, memory care, and hospice care best practices.
- Skilled in assisting with ADLs and using manual handling equipment such as Hoyer lifts safely.
- Familiarity with HIPAA regulations to ensure confidentiality of patient information.
- Experience working with individuals with autism or other neurodevelopmental disabilities; supporting intellectual disabilities is a plus.
- Capable of heavy lifting and physical tasks required for patient transfer methods while maintaining safety standards.
- Competence in first aid and CPR certification to respond effectively during emergencies.
- Excellent client communication skills to foster trust and understanding with clients.
- Ability to work independently or as part of a multidisciplinary team dedicated to providing compassionate care.
