American Traveler is seeking an experienced RN Case Manager for an inpatient care management position requiring a BSN, certified care manager credential, and recent Level 1 or comparable Level 2 Trauma experience.

Inpatient hospital setting managing a daily caseload of patients across multiple units with a variety of care needs
-
Monday through Friday schedule, 8:00 AM to 4:40 PM
-
No weekend or on-call requirements

Uses Epic EMR for documentation and order placement

Active NH or Compact RN license required
-
BSN required
-
Current BLS certification required
-
Certified Care Manager (CCM) credential required

Minimum 3 years of inpatient clinical experience required

Minimum 1 year of care management experience required

Recent Level 1 or comparable Level 2 Trauma experience within the last 2 years required

2 manager references required for consideration

Conduct initial assessments and attend interdisciplinary rounds
-
Facilitate individualized plans of care in collaboration with patients, families, caregivers, and the health care team

Communicate with medical teams and patients regarding post-discharge care needs, including home health and inpatient rehab

Coordinate with post-acute care referral sources and place orders for home health, DME, and nutritional needs in Epic

Candidates must reside more than 75 miles from the facility to be eligible for this position
