Position Title: Upfront Processing / Admission Coordinator
Summary: This position will be responsible for the coordination and management of resident information upon admission/readmission to the pharmacy system.
Educational/Work Experience
- Associate degree in health related field or 4 years direct work experience in health related field
- Previous work experience in health related field performing resident/patient admitting procedures preferred
- General knowledge of Medicare/Medicaid/Insurance Pay types, medical terminology, and brand/generic drugs needed
- Working knowledge of Microsoft Word and Excel, or like applications
- Previous long term care pharmacy experience, preferably 1-2 years

- Demonstrate superior customer service skills, both face-to-face and via the telephone
- Ability to interact with all levels of department representatives both internally and externally
- Ability to exhibit effective verbal and written communication skills
Essential Duties and Responsibilities include the following. Other duties may be assigned.
- Admission/Readmission of residents to Pharmacy System
- Process new admission/readmission information for residents being serviced by Skilled Care which includes:
- Review admission/re-admission and enter correct pay type information into RESCOT, contacting facility to obtain missing information
- Responsible for prevention of duplication of account within same facility or different facility
- Responsible for obtaining correct admission history and discharging appropriate previous information as well as pulling and canceling of potential anniversary medications to eliminate duplicate processing
- Retrieval of demographic information from facilities regarding resident insurance/discount insurance, Hospice, social security number, Medicare number, guarantor information, room number, date of birth, etc.
- Retrieval of missing facility information such as, Nursing Station, allergies, diagnosis, verifies patient medication needs, etc.
- Entering Hospice formulary when applicable as well as listing diagnosis
- Verifies Insurance/Medicaid/Hospice eligibility through Third Party Providers
- Reviews admission/re-admission and enters correct drug/ancillary profile information into RESCOT which requires the analysis, interpretation, and understanding of medication directions to ensure accuracy
- Identifies orders that can not be processed due to coverage issues, missing information, or inability to read/understand
- Completes appropriate paperwork/forms, communicating order exceptions to other SCP team members
- Coordinates forwarding fax transmissions to appropriate team members for assigned facilities
- Meets on a daily basis with appropriate team members to discuss issues surrounding new/admit residents
II. Assist Operations in Completing Daily Workflow
- Perform prescription processing duties necessitating the interpretation of brand/generic medications and the accurate entry of this information
- Perform communication related duties consisting of calling facilities with questions surrounding profiles
- Perform health information services duties that may include monthly updating, daily PTO completion, and/or report generation
- Perform basic pharmacy technician duties that may include filling prescriptions, final check scanning, and/or bagging
