Home Health Registered Nurse (RN) – Per Diem / Pay Per Visit
Come For Home Care, Inc. is seeking experienced and dependable Registered Nurses (RNs) to provide skilled home health services throughout San Diego and neighboring areas.
Position: Per DiemCompensation: Competitive Pay Per VisitSchedule: Flexible
Responsibilities:• Start of Care (SOC) assessments• Resumption of Care (ROC)• Recertifications• OASIS documentation• Skilled nursing and follow-up visits• Patient assessments and education• Medication management and wound care• Coordination with physicians and the interdisciplinary care team• Timely and accurate clinical documentation
Qualifications:• Current California RN license• Current CPR/BLS certification• Valid driver’s license and reliable transportation• Strong assessment and documentation skills• Home health experience preferred• OASIS experience strongly preferred
