The mission of PA Connecting Communities (PACC) is empowering individuals with special needs and/or needing accommodations with information and opportunities to improve their quality of life. PACC accomplishes this by developing social programs to build friendships, promoting the arts, participating in community programs, encouraging inclusion, and creating competitive employment opportunities. Our vision is to create social change through vocational (on-the-job) training programs, social and cultural activities, recreational and fitness opportunities, and remedial/academic classes for the disability community. PACC follows the principle of “Everyday Lives.”
The Direct Support Professional plays an essential role in supporting this mission by providing direct care and daily living assistance to individuals in residential settings. This position focuses on promoting independence, community participation, and overall well-being in a safe and supportive environment.
Duties include, but are not limited to:
- Assisting individuals with daily living skills, personal care, and hygiene
- Supporting participants in community outings, recreational activities, and social programs
- Promoting independence and skill development in home and community settings
- Assisting with meal preparation, light housekeeping, and medication reminders as directed
- Providing transportation to appointments, programs, and community activities
- Completing required documentation and daily reports accurately and in a timely manner
- Supporting behavioral plans and maintaining a safe, positive environment
- Assisting with lifting, transferring, and mobility needs as required
- Maintaining professional communication with supervisors and team members
Applicants must:
- Be at least 18 years of age
- Have a minimum of a high school diploma or GED equivalent
- Have at least 1 year of experience in an ID/D Residential Program preferred
- Have a valid driver’s license
- Have a reliable vehicle or means of transportation
- Be able to lift and transfer individuals as needed
- Be smart phone and computer literate
- Be able to pass Criminal Background Check, Child Abuse Clearance, and FBI check
- Be able to pass pre-employment physical and TB test
- Be able to commute to the North Hills area
