Position Summary
The Wraparound Habilitation (HAB) Provider delivers individualized, community-based habilitation services to children, adolescents, and families participating in Indiana's Wraparound Program. The HAB Provider works collaboratively with the Child and Family Team (CFT) to help youth develop daily living skills, improve emotional regulation, increase independence, and achieve goals identified in the Wraparound Plan of Care.
Services are provided primarily in the home and community settings and are designed to promote family stability, community integration, and successful functioning across home, school, and community environments.
Core Duties
- Skill Building: Deliver habilitation services according to the approved Wraparound Plan of Care, Implement interventions developed by the Child and Family Team.
- Child and Family Team: Participate as a crucial member of the Wraparound team, attending regular meetings to help implement the child’s specific care plan.
- Goal Tracking: Record and submit monthly progress reports to the Wraparound Facilitator that track the activities completed and the youth’s progress toward specific goals.
Qualifications
- Experience Requirement: You must have a minimum of two years of documented experience working with children or youth who have serious emotional disturbances.
- State Approval: You must be employed by a DMHA-approved agency or a Community Mental Health Center (CMHC) and meet the approval standards set by the state
Pay: $16.00 - $19.00 per hour
