Job Summary
We are seeking a licensed Physical Therapist for a full-time contract opportunity supporting multiple healthcare facilities in southeastern Connecticut. This position offers a competitive hourly rate, a consistent weekday schedule, and the opportunity to work within a multidisciplinary team providing rehabilitative care to individuals with physical and intellectual disabilities.
Responsibilities
- Evaluate patients with physical and/or intellectual disabilities.
- Develop and implement individualized treatment plans based on physician referrals.
- Provide individual and group therapy sessions.
- Instruct patients in activities of daily living (ADLs), independent living skills, therapeutic exercises, sensorimotor development, and functional mobility.
- Monitor patient progress and adjust treatment plans as appropriate.
- Collaborate with physicians and interdisciplinary healthcare teams.
- Maintain accurate clinical documentation and treatment records.
- Recommend, fabricate, or modify adaptive equipment when appropriate.
- Participate in treatment planning meetings and staff education.
- Support patients in community-based activities when required.
- Perform other related duties as assigned.
Qualifications
- Active Physical Therapist license in the State of Connecticut or eligibility to obtain licensure before the start date.
- Experience working with individuals with physical and/or intellectual disabilities is preferred.
- Strong assessment, treatment planning, documentation, and communication skills.
- Ability to work effectively in a secure healthcare environment.
- Must be able to meet all onboarding and background screening requirements.
Schedule:
- Monday to Friday
- 7:00 AM – 3:00 PM
- 40 hours per week
- No weekends
