Program Coordinator (Part Time)
VBP Health LLC is seeking a compassionate, organized, and dependable Program Coordinator to support individuals with developmental disabilities and oversee day-to-day program operations. The Program Coordinator will help ensure individuals receive quality support services while maintaining compliance with DDA, TennCare, and agency requirements. This role requires strong communication, problem-solving, and organizational skills, along with the ability to manage multiple responsibilities in a fast-paced environment.
Key Responsibilities
- Coordinate services and supports for individuals receiving care through VBP Health programs.
- Assist with scheduling, staffing coordination, and coverage needs for individuals served.
- Maintain accurate and organized documentation in compliance with DDA, TennCare, and agency standards.
- Support survey readiness by ensuring required documentation, monthly checks, and compliance records are maintained.
- Communicate regularly with families, conservators, support coordinators, nurses, and community partners.
- Monitor individual needs and assist with appointments, follow-ups, and community-based services.
- Respond to urgent situations involving individuals or staff and assist with resolving operational concerns.
- Assist with incident reporting, documentation review, and follow-up actions when necessary.
- Help oversee staff performance, training compliance, and certification tracking.
- Conduct regular audits of documentation and program files to ensure compliance and quality assurance.
- Support onboarding and orientation of new staff members.
- Participate in care planning meetings and assist with implementation of person-centered supports.
- Promote individual rights, dignity, independence, and community integration.
Qualifications
- Experience working with individuals with developmental disabilities preferred.
- Knowledge of DDA, TennCare, ECF CHOICES, or community-based services preferred.
- Understanding of survey processes, compliance expectations, and documentation standards.
- Strong organizational and time-management skills.
- Ability to multitask and work independently in a fast-paced environment.
- Excellent verbal and written communication skills.
- Professional and compassionate approach when working with individuals, families, and staff.
- Experience with scheduling, documentation, or program coordination preferred.
- Proficient in Microsoft Office and basic computer systems.
Physical Requirements
- Ability to travel locally as needed for meetings, appointments, or individual support.
- Ability to respond to urgent operational situations when necessary.
