Job Summary
We are seeking a compassionate and experienced Psychiatric-Mental Health Nurse Practitioner (PMHNP) to deliver high-quality mental health care to residents in skilled nursing and long-term care settings. This role is ideal for a clinician with a strong foundation in geriatric psychiatry, a collaborative mindset, and a genuine commitment to enhancing the mental health and overall well-being of older adults.
Key Responsibilities
- Conduct comprehensive psychiatric evaluations and ongoing mental health assessments.
- Develop, implement, and regularly update individualized treatment plans in collaboration with interdisciplinary teams.
- Diagnose and manage a broad spectrum of psychiatric conditions, including depression, anxiety, and dementia-related behavioral disturbances.
- Prescribe, monitor, and adjust psychotropic medications, evaluating therapeutic outcomes and potential side effects.
- Coordinate referrals for psychotherapy, behavioral health services, and additional supportive care as needed.
- Collaborate closely with physicians, nursing staff, social workers, and facility personnel to ensure continuity and quality of care.
- Maintain accurate, timely, and compliant documentation in accordance with clinical, regulatory, and organizational standards.
- Provide education and guidance to staff and caregivers on behavioral health management, best practices, and crisis prevention.
- Ensure adherence to all applicable state, federal, and facility-specific regulations and policies.
Qualifications
- Education: Master’s or Doctoral degree in Nursing from an accredited program, with PMHNP certification.
- Licensure: Active, unrestricted Nurse Practitioner license and DEA registration in the state of practice.
- Experience: Prior experience in geriatric psychiatry, skilled nursing facilities, or long-term care settings strongly preferred.
