Position Summary:
The Case Manager is responsible for ensuring pregnant and parenting teenage clients are provided with a comprehensive program of services based on a professional assessment of their needs and availability of community resources. Moreover, the Case Manager will provide support to families parenting young children in the areas of breastfeeding, attachment, child development and other related topics using the Parents As Teachers curriculum. This position is fieldwork conducted primarily through in-person home visits and other pertinent locations (school, clinic, etc.).
Essential Duties:
- Meets with clients to conduct a comprehensive psychosocial assessment; assesses and identifies strengths, challenges and needs; develops quarterly service plans.
- Helps clients locate community resources; refers clients and/or acts as advocate in obtaining services for them; acts as liaison between service providers, clients and others involved in clients’ lives.
- Assists clients in locating, enrolling in, and making progress in school toward high school completion.
- Maintains frequent contact with clients through home visits, office and field appointments and telephone conversations to follow through with services; works closely with clients and families to motivate and overcome resistance using Motivational Interviewing techniques.
- Develops effective relationships to promote positive pregnancy outcomes. Increases parenting knowledge and skills, educates on a wide variety of topics including child development, reproductive healthcare and family planning, home and child safety, and teen and well-baby health.
- Provides transportation for clients as needed to attend appointments to ensure they receive needed services. Provides transportation as needed to attend client events at the office and offsite.
- Facilitates/co-facilitates group meetings for pregnant and parenting clients in various settings.
- Identifies, consults on, and reports possible instances of child abuse.
- Maintains a variety of records relating to clients and services; prepares routine and special reports.
- Maintains a working knowledge of services and resources available in the community; maintains effective working relationships with DPSS, DCFS, schools, and other resources in the community to facilitate advocacy and services for clients.
- Maintains professional skills and knowledge through in-service programs, reading, professional development trainings online and in the community, and professional contacts in the field.
- Performs a wide variety of related duties depending on client’s needs (e.g., screens all children for developmental delays, surveys clients on specific topics for research purposes, provides education around healthy relationships and pregnancy, etc.).
Requirements:
- BA Degree in Social Work, Psychology or related field.
- Minimum of two years of case management work experience. Prior experience with teenage/multi-cultural clients is preferred.
- Parents as Teachers certification is a plus, otherwise, completion of comprehensive PAT training within three months of hire.
- Essential characteristics for mastering Parents as Teachers five core competency areas (family support and parenting education; child and family development; human diversity within family systems; health, safety and nutrition; and relationship between families and communities).
- Working knowledge of early childhood education and the developmental milestones of babies and toddlers, typically resulting from completion of any combination of formal education, seminars, and work experience in related areas.
- Working knowledge of current welfare policies, child abuse reporting laws, Agency/State program policies, school laws relating to teenagers, and the legal issues pertaining to teens, parenting, custody, childcare, marital/domestic violence, welfare and public assistance.
- Effective written/oral communication and engagement skills to develop effective rapport and personal relationships and to counsel and mentor young people.
- Ability to relate well with and develop effective and positive working relationships with teenage clients and families from different cultures living at poverty levels.
- Crisis intervention skills (e.g., conflict resolution, domestic violence intervention).
- General computer skills and ability to learn and use a data system.
- Valid California driver’s license, and an insured automobile to drive to clients’ homes and to transport clients as necessary.
- Bilingual English/Spanish Required.
