Cobble Hill is seeking a dedicated RN – Maternal Health/Postpartum Nurse to provide high-quality in-home care and support to mothers, newborns, and families throughout the prenatal and postpartum journey.
This role is ideal for a Registered Nurse passionate about maternal-child health, patient education, and building meaningful relationships with families during one of life's most important transitions.
Schedule

Monday–Friday | 9:00 AM–5:00 PM

Responsibilities
-
Conduct clinical assessments of pregnant women, postpartum mothers, newborns, and infants in the home setting
-
Monitor maternal vital signs and assess infant growth and developmental milestones

Educate families on:

Breastfeeding and lactation support
-
Safe sleep practices
-
Newborn care and parenting skills
-
Nutrition and postpartum recovery
-
Family planning and maternal wellness
-
Coordinate patient care and collaborate with interdisciplinary healthcare teams
-
Identify changes in patient condition and communicate findings appropriately

Maintain accurate and timely clinical documentation
-
Provide emotional support and guidance to patients and families
Qualifications
-
Active RN license in New York State
-
1–2 years of clinical experience in:

Maternal/Child Health
-
Labor & Delivery
-
Pediatrics
-
Community Health
-
Postpartum Care
-
Current BLS certification required
-
PALS certification required
-
Lactation Specialist certification preferred

Strong communication, assessment, and patient education skills
-
Homecare experience is a plus
