We are seeking a passionate, bilingual (Spanish) Prevention Services Coordinator to support LGBTQ+ young people through case management, sexual health education, supportive counseling, and leadership development programs.
This role is ideal for a professional who is committed to health equity, HIV prevention, youth development, and serving LGBTQ+ communities. The Prevention Services Coordinator will provide direct client services, supervise program staff, coordinate prevention initiatives, and help connect young people to critical support services.

Key Responsibilities:
Staff Supervision & Program Coordination
- Supervise and support two part-time LGBTQ+ Peer Leaders and one full-time Care Coordinator.
- Participate in staff recruitment, onboarding, training, and professional development.
- Provide weekly supervision, performance feedback, and skill development coaching.
- Maintain employee records and support quality program implementation.
- Ensure services are delivered efficiently, effectively, and in alignment with program goals.
Direct Client Services
- Provide strengths-based, client-centered case management services to LGBTQ+ individuals at risk for HIV transmission or living with HIV.
- Conduct comprehensive behavioral risk assessments and develop individualized service plans.
- Provide supportive counseling and ongoing client engagement.
- Maintain accurate and timely client documentation and records.
Sexual Health & HIV Prevention
- Promote sexual health education and evidence-based prevention strategies.
- Support client engagement in HIV prevention and treatment services, including PrEP, PEP, and HIV care.
- Conduct rapid HIV testing, confirmatory HIV testing, and syphilis testing as required.
- Support client retention and adherence to sexual health-related medical interventions.
Community Resource Navigation
- Connect clients with essential support services, including:
- Housing assistance
- Food resources
- Insurance navigation
- Legal name change assistance
- Other community-based services
Data Management & Reporting
- Maintain electronic and paper records in compliance with agency and funding requirements.
- Track and report monthly program statistics and client outcomes.
- Document outreach activities, community advisory board participation, and service linkages.
Additional Responsibilities
- Provide clinical and programmatic support for events and activities as needed.
- Perform other duties assigned by the Regional Program Manager.
Qualifications Required
- Bachelor's degree in Social Work, Human Services, Public Health, Mental Health Counseling, Psychology, or a related field.
- Minimum of 2 years of experience in sexual health prevention, HIV prevention, case management, program coordination, or related services.
- Minimum of 2 years of supervisory or leadership experience.
- Bilingual English/Spanish.
- Strong organizational, communication, and interpersonal skills.
- Ability to work independently and collaboratively in a team environment.
- Proficiency with Microsoft Office 365, including Outlook, Word, Excel, Teams, and SharePoint.
- Reliable transportation and ability to travel throughout Nassau and Suffolk Counties.
- Availability to work occasional evenings and weekends.
Preferred
- Master's degree in Social Work, Mental Health Counseling, Public Health, or a related field.
- Experience working with LGBTQ+ youth and young adults.
- Knowledge of HIV prevention, PrEP, PEP, U=U, sexual health education, and harm reduction approaches.
- Familiarity with community resources throughout Nassau and Suffolk Counties.
- Professional and/or lived experience within BIPOC and/or TPOC communities.
