Position Overview
Seeking qualified professionals for a Permanent Social Worker (LBSW) or Care Manager position within the Medical Surgical department. This role focuses on coordinating patient care, discharge planning, resource utilization, and ensuring patients receive appropriate services throughout their hospital stay while promoting quality, safety, and positive health outcomes.
Candidates will work collaboratively with physicians, nursing staff, patients, families, and interdisciplinary healthcare teams to assess patient needs, develop individualized care plans, and coordinate appropriate levels of care and community resources.
Compensation
- Social Worker (LBSW): Starting at $24.77/hour
- Care Manager: Starting at $30.11/hour
- Compensation based on education and experience
- Relocation assistance available up to $5,000 (discussed during interview)
Social Worker (LBSW) Responsibilities
- Coordinate individualized patient care plans throughout hospitalization.
- Perform psychosocial assessments and identify patient and family support needs.
- Assist with discharge planning and transitions of care.
- Connect patients with community resources and supportive services.
- Collaborate with physicians, nurses, case managers, and interdisciplinary teams.
- Advocate for patients while promoting quality, safety, and efficient healthcare delivery.
- Monitor patient progress and update care plans as needed.
- Maintain respectful, compassionate communication with patients and families.
- Support medical necessity and appropriate level-of-care planning.
- Document assessments, interventions, and care coordination activities accurately.
Care Manager Responsibilities
- Assess, plan, implement, monitor, and evaluate individualized patient care plans.
- Coordinate services to ensure timely and appropriate healthcare delivery.
- Evaluate medical necessity of admissions and appropriate levels of care.
- Facilitate communication between patients, families, providers, and healthcare teams.
- Promote positive patient outcomes through efficient care coordination.
- Monitor utilization of healthcare services and support safe discharge planning.
- Collaborate across departments to ensure continuity of care.
- Maintain patient dignity, respect, and excellent customer service throughout the care process.
Social Worker (LBSW) Requirements
- Bachelor's Degree in Social Services required.
- Active Kansas LBSW License required.
- Hospital-based social work experience preferred.
- Strong communication, interpersonal, and organizational skills.
- Ability to collaborate effectively within an interdisciplinary healthcare environment.
Care Manager Requirements
- Associate Degree in Nursing or Diploma from an accredited RN program required.
- BSN preferred.
- Active Kansas Registered Nurse License required.
- Minimum 2 years of acute-care Medical Surgical nursing experience required.
- Strong care coordination and discharge planning abilities.
