Vista Living Care is looking for a compassionate, organized, and clinically strong Clinical Coordinator to support our Memory Care homes and Home Care services. This is an excellent opportunity for an experienced healthcare professional who enjoys building relationships, coaching caregivers, partnering with families, and improving quality of care.
Unlike a traditional Director of Nursing role, this position focuses on clinical coordination, quality oversight, and supporting our care teams in delivering exceptional, person-centered care.

What You'll Do
As our Clinical Coordinator, you'll help ensure our elders and clients receive the highest quality care by:
- Completing elder and client assessments
- Developing and updating individualized care plans
- Monitoring changes in condition and coordinating appropriate interventions
- Overseeing medication management and clinical documentation
- Coordinating admissions, hospital returns, hospice enrollments, and discharges
- Monitoring wounds and other clinical concerns
- Investigating incidents and identifying opportunities for quality improvement
- Coaching and supporting Care Partners, Medication Assistants, and Home Care staff
- Partnering with physicians, pharmacies, hospice providers, and other healthcare professionals
- Building positive relationships with elders, families, and team members
- Helping ensure compliance with Department of Health regulations and company policies
What We're Looking For
Required:
- Current LPN or RN license preferred
- Experience in assisted living, memory care, home care, long-term care, hospice, or senior living
- Strong assessment and clinical decision-making skills
- Experience with care planning and medication management
- Excellent communication and organizational skills
- Valid driver's license and reliable transportation
Preferred:
- Dementia care experience
- Experience training or mentoring caregivers
- Knowledge of New Mexico assisted living regulations
- Experience with electronic health records

Pay: $60,000.00 - $80,000.00 per year
