We are seeking a dynamic and experienced RN Nurse to lead our Post-Acute Care team at Agave Grove. Job Title: Nurse
Salary: $39-$45 (Depending on License and years of experience)
Schedule: 12 Hour shifts, 6am-6pm, rotating weekends. Skilled Unit
Duties
- Oversee clinical operations across multiple departments, including long-term care, memory care, and acute post-acute services.
- Ensure compliance with all healthcare regulations, including HIPAA, OSHA standards, and state-specific licensing requirements.
- Coordinate interdisciplinary team efforts to develop individualized care plans that meet residents’ physical, emotional, and psychological needs.
- Manage patient assessments, medical documentation, medication administration, and medical records using EMR (Electronic Medical Records) systems such as Epic or Cerner.
- Conduct quality assurance audits and participate in utilization review and management to optimize patient outcomes and resource utilization.
- Lead and supervise nursing staff, including scheduling, performance evaluations, and professional development to ensure high standards of patient care.
Qualifications
- Valid RN licensure with current state registration
- Extensive experience in post acute care settings or acute care such as ICU or Level I Trauma Centers; critical care experience highly valued.
- Proven supervisory or nurse management experience within long-term care, assisted living, or skilled nursing facilities.
- Strong knowledge of physiology, medical terminology, infection control practices, and medical imaging procedures.
- Experience working with individuals with developmental disabilities or dementia care is a plus.
- Excellent leadership skills with the ability to motivate teams and foster a positive work environment.
