The Behavioral Health Clinician (BHC) will be a core member of our Collaborative Care Model (CoCM) team. We partner with primary care, family medicine and pediatric medicine practices to address behavioral health needs using a population health, interdisciplinary team model. The team will also include the patient’s medical provider, Behavioral Health Care Manager, and a psychiatric consultant.
The BHC is responsible for providing brief, evidence-based clinical support and interventions for patients enrolled in the Collaborative Care Program. This includes Initial Evaluation, Differential Diagnosis, treatment planning, and treatment delivery utilizing evidence-based interventions.
The successful candidate for this position should be able to carry out effective clinical interventions, coordinating closely with fellow CoCM team members. In some cases, the BHC will provide guidance and oversight for their team’s BH Care Manager.
This position will be full-time (40 hours per week) with typical days and hours being Monday – Friday, 8:30 am-5pm with a 30 minute lunch. This position will be based in Chelmsford, MA. Services are typically provided on-site at our medical partner’s clinics and remotely via secure telehealth.
BHC DUTIES AND RESPONSIBILITIES:
● Support the mental and physical health care of patients on an assigned patient caseload. Closely coordinate care with the patient’s medical provider and, when appropriate, other mental health providers.
● Screen and assess patients for common mental health and substance use disorders using validated screening tools.
● Facilitate patient engagement and follow-up care.
● Provide short-term, evidence-based therapeutic interventions such as CBT, Motivational Interviewing, and Solution-Focused Brief Therapy.
● Provide patient education about common mental health and substance use disorders and the available treatment options using approved psychoeducation materials.
● Systematically track treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
● Work with patients to assess and address identified Social Needs such as food insecurity, transportation, and housing needs.
● Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments (e.g., problem-solving treatment or behavioral activation) as clinically indicated.
● Coordinate with larger systems on patient’s behalf, including when appropriate with schools, other medical providers, and/or family members.
● Participate in regularly scheduled (weekly) caseload consultation with a psychiatric consultant and communicate resulting treatment recommendations to the patient’s medical provider. Caseload consultation reviews may be conducted by telephone, video, or in person.
● Track patient follow up and clinical outcomes using a Registry. Document in-person and telephone encounters in the registry and use the system to identify and re-engage patients.
● Document patient progress and treatment recommendations in EHR and other required systems to be shared with medical providers, psychiatric consultants, and other treating providers.
● In consultation with the medical provider and the psychiatric consultant, facilitate treatment plan changes for patients who are not improving as expected and/or who may need more intensive or specialized mental health care.
● Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are preparing to be discharged from the caseload.
● Coordinate with existing clinic billing team members to bill for services.
TRAINING AND LICENSURE REQUIREMENTS:
● Master’s Degree in mental health related field
● Working towards independent licensure or holds an unrestricted License issued by the State of MA Board of Allied Mental Health Providers, in good standing
● Helios will provide extensive support and training specific to the collaborative care model and program
While previous experience in integrated care is not required, strong candidates will have 2+ years of clinical experience as a therapist and some prior experience in integrated models of behavioral healthcare.

● Demonstrated ability to collaborate and communicate effectively in a team setting.
● Ability to maintain effective and professional relationships both with patients and other members of the care team.
● A desire to improve behavioral health access and outcomes through Helios’s innovative programs.
● Ability to work with patients virtually as well as in person.
● Working knowledge of evidence-based therapeutic models of care.
● Understanding of HIPAA regulations and ability to work while maintaining confidentiality and privacy in accordance with governing HIPAA regulations.
● Seeking candidates fluent in Spanish and capable of delivering care in both English and Spanish
● Past clinical experience working with pediatric, adolescent and family systems is ideal for this position.

The BHC will have designated office space at our partner clinics to conduct their duties. PAY RANGE:
Annual salary of $80,000 to $95,000 plus benefits package including paid time off.
This position is eligible for benefits during the probationary period. Paid time off accrues from the first day of employment.
