About the Role:
We are seeking a dedicated and experienced Physical Therapist to join our interdisciplinary team. As a Physical Therapist at The Orchards at Harper Woods, you will be responsible for creating and implementing comprehensive rehabilitation plans that cater to the unique needs of our residents. Your expertise will enable you to assess, treat, and educate patients with a wide range of diagnoses and conditions. You will also collaborate with our healthcare team to ensure seamless communication and coordination of care.
Responsibilities:
Assess and treat patients with a variety of physical disabilities, injuries, and conditions
Develop and implement individualized rehabilitation plans that promote optimal recovery and independence
Conduct evaluations, progress reports, and discharge summaries
Provide education and training to patients, families, and caregivers on proper techniques and strategies for maintaining strength, flexibility, and mobility
Collaborate with our healthcare team to ensure comprehensive care and coordination
Maintain accurate and timely records of patient assessments, treatments, and progress
Requirements:
Degree in Physical Therapy
Current licensure as a Physical Therapist in the state of Michigan
Minimum of 0 years of experience in acute care, post-acute care, or long-term care settings
Strong communication and interpersonal skills
Ability to work effectively as part of an interdisciplinary team
