Job Overview
BLEHEALTH is seeking an experienced Registered Nurse (RN) Clinical Consultant to provide clinical oversight and support for our Enhanced Care Management (ECM) program. Under the supervision of the Clinical Director, this role works closely with Lead Care Managers, Home Care Aides, and Housing Navigators to coordinate care for members with complex medical, behavioral health, and social needs.
The RN Clinical Consultant serves as both a clinical leader and direct care team member, providing guidance to staff while also managing high-acuity cases requiring advanced clinical intervention. The ideal candidate has experience in care coordination, case management, interdisciplinary collaboration, and working with vulnerable populations.
This is a full-time position based in our corporate office in Pomona, California, with a regular schedule of Monday through Friday, 8:00 a.m. to 5:00 p.m.
Responsibilities
- Provide clinical oversight and consultation to care management staff.
- Manage and support high-acuity members with complex medical, behavioral health, and social needs.
- Review assessments and Health Risk Assessments (HRAs) and provide clinical recommendations.
- Develop, review, and approve member-centered care plans and care plan updates.
- Collaborate with Lead Care Managers, Home Care Aides, Housing Navigators, Primary Care Providers, Behavioral Health Providers, specialists, hospitals, managed care plans, and community organizations to coordinate care.
- Support medication management, reconciliation, and adherence.
- Assist with hospital admissions, discharges, and transitions of care to reduce readmissions.
- Participate in case conferences, interdisciplinary care team meetings, and complex case reviews.
- Provide health education related to chronic disease management, preventive care, and treatment adherence.
- Conduct chart reviews and ensure timely, accurate documentation.
- Train and support staff to maintain quality standards and regulatory compliance.
- Use data and quality measures to evaluate outcomes and improve service delivery.
- Participate in community outreach activities, including visits to member homes, hospitals, shelters, and other community settings as needed.
- Support quality improvement initiatives and perform other duties as assigned.
Required Qualifications
- Current California Registered Nurse (RN) license in good standing.
- Bachelor's Degree in Nursing (BSN) required; Master's degree preferred.
- Minimum four years of nursing experience in home health, primary care, care management, population health, or a related healthcare setting.
- At least one year of supervisory, administrative, or clinical leadership experience.
- Knowledge of care coordination, chronic disease management, medication reconciliation, and quality improvement.
- Experience with EMR/EHR systems and clinical documentation.
- Strong communication, leadership, and organizational skills.
- Valid California driver's license, reliable transportation, and willingness to travel locally.
- Current CPR certification, negative TB clearance, and ability to pass a Live Scan background check.
Preferred Qualifications
- Experience with Enhanced Care Management (ECM), Medi-Cal, Managed Care, Home Health, or Population Health programs.
- Experience serving high-risk, high-acuity, or underserved populations.
- Bilingual skills are a plus.

Pay: $45.00 - $48.00 per hour
