Overview
We are seeking a compassionate and motivated Nurse Practitioner (APRN) to provide high-quality medical care for patients in short-term rehabilitation and long-term care facilities. The ideal candidate will be skilled in managing chronic conditions, conducting routine follow-up visits, coordinating care, and promoting positive patient outcomes.
Duties
- Conduct comprehensive patient assessments, physical examinations, and follow-up visits to evaluate health status and identify care needs.
- Diagnose and manage acute and chronic medical conditions, including reviewing laboratory results, diagnostic tests, and medication regimens.
- Develop and implement individualized treatment plans and administer appropriate medications and therapies.
- Provide patient and family education regarding disease management, preventive care, medications, and healthy lifestyle choices.
- Collaborate with physicians, facility staff, specialists, therapists, and families to coordinate high-quality patient care.
- Maintain accurate and timely medical documentation while participating in care coordination, quality improvement, and compliance initiatives.
Candidates should possess a diverse skill set that includes but is not limited to:
- Proficiency with EMR/EHR systems such as Epic, Athenahealth, Cerner, eClinicalWorks and Tebra.
- Experience in critical care settings including ICU, ER, or Level I/II trauma centers.
- Knowledge of medical terminology and coding (ICD-9/ICD-10/CPT).
- Familiarity with various treatment modalities including botulinum toxin treatment, dermal fillers, esthetic laser treatment, and pain management techniques.
- Strong background in pediatrics, geriatrics, hospice & palliative medicine, and behavioral health.
- Skills in performing procedures such as suturing, catheterization, phlebotomy, spinal taps, and IV insertion.
- Ability to manage chronic conditions through case management and health coaching.
- Experience with infectious disease care and outpatient services.
- Understanding of senior care practices including Alzheimer’s care and memory care.
