Job Summary:

Responsibilities:
- Provide direct patient care, including wound care, vital signs, and medication administration
- Develop and implement individualized care plans in collaboration with the healthcare team
- Monitor and document patient progress and report any concerns or changes to the healthcare team
- Participate in patient education and support to promote optimal health outcomes
- Maintain accurate and detailed records of patient care and interactions
- Collaborate with other healthcare professionals to ensure seamless and effective patient care
Requirements:
- Current LPN license in the state of Connecticut
- Minimum 1 year of experience in a healthcare setting
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
