Camden, TN & Surrounding Counties
Schedule: Monday–Friday | 8:00 AM – 4:30 PM- Regular weekday patient visits with participation in a rotating on-call schedule, including evenings and weekends. Daily hours vary based on patient needs, travel, and documentation.

Key Responsibilities
-
Evaluate patients and develop individualized treatment plans based on physician orders
-
Provide skilled physical therapy services in accordance with the physician’s plan of care and state regulations
-
Help patients improve mobility, strength, balance, and functional independence
-
Educate patients, families, and caregivers on treatment plans, home exercise programs, and safety techniques
-
Recommend adaptive equipment and home modifications to promote safety and independence
-
Collaborate with physicians and the interdisciplinary care team to achieve optimal patient outcomes
-
Complete accurate and timely clinical documentation and care plan updates
-
Supervise Physical Therapist Assistants (PTAs) in accordance with company policy and state regulations
-
Encourage and motivate patients throughout their rehabilitation journey
Qualifications
-
Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA)
-
Current Physical Therapist (PT) license in the state of Tennessee
-
Home Health experience preferred, but not required
-
Two (2) years of experience in an acute care or similar clinical setting preferred
-
Current CPR and First Aid certification
-
Valid driver’s license, reliable transportation, and proof of automobile insurance
