Lewistown Home Health, a leading provider of home health and hospice care, has a great full time opportunity for a friendly, compassionate and skilled Licensed Practical Nurse to join our team of home care clinicians. LPN Responsibilities (not all inclusive):
- The LPN will make home visits to provide skilled nursing care for select chronic and sub-acute patients.
- The LPN will ensure quality and safe delivery of home health services to patients by implementing the plan of care initiated by the home health RN/Patient Care Advocate.
- The LPN will assist patients with activities of daily living while teaching appropriate self-care and administering prescribed medications and or treatments, including venipunctures and/or wound care, as ordered by a physician and permitted by the State and agency policies and procedures.
- The LPN will participate in coordination of home health services, appropriately reporting the needs for other disciplines to the home health RN/Patient Care Advocate.
- The LPN will assess and provide patients and families education and information pertinent to diagnosis and care plan.
- The LPN will provide accurate and timely documentation consisted with the plan of care and participate in nursing team conferences.
- The LPN will provide input into patient care planning and interpret the patient's nursing needs and response to therapy.
- The LPN will lift and move patients in the course of care and responsibilities.
- The LPN will perform other duties as appropriately assigned.
