All Valley Home Health Care Inc. is looking for a compassionate and dependable Registered Nurse (RN) to join our growing Home Health team. This is a part-time, paid per visit position offering flexibility while allowing you to provide exceptional one-on-one care to patients in the comfort of their homes.

Responsibilities
- Complete skilled nursing visits in patients' homes.
- Perform Start of Care (SOC), Recertification, Resumption of Care (ROC), Routine Skilled Nursing, Supervisory, and Discharge visits as assigned.
- Conduct comprehensive nursing assessments and develop individualized plans of care.
- Provide skilled nursing services including wound care, medication management, catheter care, disease management, IV therapy, and patient education.
- Collaborate with physicians and the interdisciplinary team to ensure quality patient care.
- Educate patients and caregivers on disease management, medications, safety, and home care.
- Complete accurate and timely electronic documentation in accordance with Medicare and agency guidelines.
Qualifications
- Current Michigan Registered Nurse (RN) license.
- Minimum of one (1) year of nursing experience required.
- Home Health experience preferred, but willing to train the right candidate.
- OASIS experience is a plus.
- Current CPR/BLS certification.
- Valid Michigan driver's license, reliable transportation, and current auto insurance.
- Strong communication, organizational, and critical thinking skills.
