The LTC Nurse provides a one-time thorough (hands-off) assessment to establish qualification for in-home services through their respective insurance carrier. The assessment includes but is not limited to documenting the clients medical history and medications, assess cognitive and mental status, assess the home environment for safety, observe ADL/IADLs, determine current service plan availability.
- Graduate of an accredited school of nursing, Baccalaureate degree preferred
- Current licensure as a registered nurse in the State of Illinois.
- Minimum of two years of nursing in an acute care or home care setting preferred.
- CPR certification preferred, not required.
- Current Illinois driver’s license with proof of automobile insurance on personal vehicle for business use or a l t e r n a t e reliable transportation.
- Must be empathetic, non-judgmental, tactful, responsible, adaptable, organized, and committed to the purpose and objectives of the organization.
- Must have excellent communications skills
- Must have emotional and mental maturity necessary for establishing and maintaining good working relationships with patients, patient’s families, and Agency personnel.
- Must understand the team approach and be a supportive team member.
