Depending on the individual you support, services may include assistance with daily activities, personal care, household tasks, transportation, community activities, companionship, and other individualized supports.

Responsibilities
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and manual handling techniques such as Hoyer lifts.
- Monitor patients’ vital signs and observe for changes in health status, reporting findings accurately while maintaining HIPAA compliance to protect client privacy.
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption to promote health and well-being.
- Support patients with medication administration following care plans and ensuring safety protocols are adhered to at all times.
- Provide companionship and emotional support to clients with dementia, Alzheimer’s, autism, or other neurodevelopmental or developmental disabilities, fostering a respectful and engaging environment.
- Maintain cleanliness of client spaces through regular cleaning routines aligned with home & community care standards.
- Assist with transportation needs for appointments or social activities using personal transportation assistance skills.
Qualifications
- Prior experience working in group homes, nursing homes, assisted living facilities, or home health environments is highly valued.
- Knowledge of patient observation techniques, vital signs monitoring, medication administration, and emergency response procedures such as CPR and first aid.
- Experience working with individuals with dementia, Alzheimer’s disease, autism spectrum disorder (ASD), intellectual disabilities or neurodevelopmental disorders is preferred.
- Ability to perform heavy lifting safely and utilize patient transfer methods like Hoyer lifts when necessary.
- Strong communication skills for effective client interaction and documentation of care activities while respecting HIPAA regulations.
- Demonstrated ability to handle manual tasks including cleaning duties and assisting with ADLs in a compassionate manner.
- Certification in CPR/First Aid is a plus; ongoing training opportunities are available for professional growth.
