We are seeking a experienced Hospice Clinical Liaison to join our hospice team. The Liaison will directly shape our growth, reputation, and culture. The territory for this position is Summit, Portage and the surrounding area.
Responsibilities:
- Develop hospice referrals and admissions across hospitals, SNFs, ALFs, physician offices, home health agencies, and community partners.
- Identify hospice-appropriate patients early and help improve referral timing and conversion.
- Educate referral partners on hospice eligibility, levels of care, and when hospice is appropriate (and when it's not).
- Partner closely with Intake and Clinical teams to ensure timely evaluations and smooth admissions.
- Building relationships with case managers, discharge planners, social workers, nurses, physicians, and administrators.
- Act as the frontline voice of the market-- bringing insight on referral trends, barriers, and competitive activity.
- Help leadership understand what's working, what's not, and where to focus next.

What we are looking for:
- 2+ years in healthcare sales, liaison, or account management, required
- LPN License
- Comfort discussing serious illness and end-of-life care with professionalism and empathy
- Strong communication, organization, and follow- through
- Ability to work independently and manage a territory
Preferred
- Hospice or home health sales experience
- Existing relationships in the local healthcare community
