Responsibilities
- Conduct comprehensive patient evaluations to determine functional limitations and develop effective treatment plans aligned with clinical standards.
- Implement evidence-based physiotherapy techniques such as trigger point therapy, orthopedics, neurology interventions, and geriatrics care to facilitate optimal recovery.
- Monitor patient progress regularly, adjusting care plans as needed to ensure continuous improvement and adherence to discharge planning protocols.
- Collaborate closely with healthcare professionals including physicians, nurses, occupational therapists, and social workers to coordinate patient care strategies.
- Maintain accurate and detailed clinical documentation in electronic health records (EHR) systems in compliance with medical terminology and documentation standards.
- Educate patients and their families on treatment procedures, home exercises, and community resources to promote ongoing rehabilitation outside clinical settings.
- Participate in interdisciplinary team meetings to discuss patient progress, discharge planning, and care plan development for acute care, inpatient, outpatient, or home health scenarios.
Requirements
- Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction.
- Proven experience practicing in a physical therapy role within outpatient clinics, hospitals, inpatient units or home health environments.
- Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards necessary for accurate record keeping.
- Demonstrated ability to develop individualized treatment plans for diverse populations including pediatrics, geriatrics, orthopedics, neurology patients and those requiring outpatient rehabilitation.
- Experience working with electronic health records (EHR) or electronic medical records (EMR) systems for documentation and patient management.
- Excellent client communication skills with the ability to motivate patients through education and compassionate care delivery.
- Familiarity with discharge planning processes and working collaboratively with healthcare teams to ensure seamless patient transitions from hospital to home or community settings.
- Prior outpatient rehabilitation center experience or outpatient clinic experience is highly desirable; inpatient or hospital experience is a plus.

Pay: $52.33 - $75.02 per hour
