We are seeking a motivated and compassionate Intake Nurse Licensed Practical Nurse (LPN) to join our dynamic team. In this vital role, you will be the first point of contact for new patients, conducting thorough assessments and coordinating care plans to ensure seamless transitions from hospital or clinic settings to home-based care. Your expertise in acute and post-acute care environments will help us deliver exceptional patient service while maintaining compliance with healthcare regulations. This position offers an exciting opportunity to make a meaningful difference in patients’ lives through proactive intake management and clinical evaluation.
This is an in-person position, and team members are required to report to the office each day
Responsibilities
- Office position focused on managing referrals from hospitals, physician offices, and other healthcare providers. The role involves coordinating referral intake, communicating with referral sources, and ensuring patients are processed accurately and efficiently
- Receive, track, document, and communicate patient referrals, physician orders, and insurance authorizations
- Demonstrate critical thinking and sound decision-making skills throughout the intake process
- Maintain accurate documentation and ensure confidentiality of medical records in accordance with HIPAA regulations
- Coordinate referral development, consultation, and patient care communication
- Educate patients and families regarding healthcare procedures, medications, and treatment plans
- Collaborate with physicians, clinical staff, and healthcare professionals to ensure quality patient care
- Utilize electronic medical record (EMR) systems efficiently and accurately
Qualifications
- Current West Virginia LPN license
- Strong understanding of medical terminology and medical records
- Excellent verbal and written communication skills
- Strong organizational skills and attention to detail
- Ability to prioritize tasks and manage time effectively
- Ability to work independently and as part of a team
- Experience with electronic medical records preferred
