Position Overview
We are seeking a Hybrid Medical Assistant (Clinical & CCM) to support both clinic operations and Chronic Care Management (CCM). This role combines direct patient care with care coordination, chart review, and patient outreach.
Qualifications
- Certified Medical Assistant (CMA, RMA, or equivalent)
- Primary care experience preferred
- CCM, HCC, or HEDIS experience preferred
- Experience with eClinicalWorks (eCW) preferred
- Strong communication, organizational, and multitasking skills
- Bilingual English/Spanish is a plus
Responsibilities
- Obtain and document vital signs and patient histories
- Assist providers with exams and procedures
- Perform injections, EKGs, spirometry, ear lavages, RetinaVue exams, and urinalysis
- Conduct chart reviews to identify and address HCC and HEDIS gaps
- Coordinate patient care, follow-up calls, referrals, and appointment scheduling
- Process prescription refills and prior authorizations
- Perform telephone triage according to practice protocols
