American Traveler is seeking a Physical Therapist for a 13-week home health assignment in a rural, multi-county service area, requiring at least 1 year of experience and active BLS certification.

Home health care setting serving four counties (Noble, Lagrange, Steuben, and DeKalb), primarily rural with mid-sized towns
-
Caseload equivalent to approximately 4–6 patient visits per day using a point-based tracking system
-
Common cases include elderly falls and fractures

Responsibilities include Oasis Assessments and Start of Care cases

Charting system: Homecare Homebase (HCHB)
-
Tablet used for time and visit tracking

Schedule: Monday–Friday, 8:00 AM–5:00 PM; day shift, 8-hour shifts

Minimum 1 year of physical therapy experience required

Active PT state license or Compact (NLC) licensure required; pending licenses will not be accepted — license required if applicable

Active BLS (AHA) certification required; pending certifications will not be accepted

Valid driver's license required for consideration

2 professional references required: one supervisor from within the last 12 months worked, and one peer or supervisor from within the last 3 years worked, including dates of employment and eligibility for rehire

12-hour orientation provided

Attire: business casual or hunter green scrubs with a white or black top
