WHERE YOUR EXPERTISE LIVES- Complete body assessments on all new admissions and implement preventive interventions
- Measure and document all wounds weekly with accurate, timely clinical notes
- Monitor PCC Dashboard daily for new skin areas and ensure appropriate nursing follow-up
- Initiate and perform wound care following center protocols and physician orders
- Communicate regularly with patients and families regarding wound and skin conditions
- Collaborate with MDS Coordinators to ensure accurate coding and supporting documentation
- Participate in weekly at-risk meetings and monthly QAPI wound trend reporting
- Partner with CNA and nursing staff on skin integrity identification and wound prevention education
- Round with wound care physicians and center leadership to assess and revise plans of care
- Evaluate supply usage and wound care supply accuracy with nursing leadership monthly
WHAT YOU BRING TO THE BEDSIDE
- A current RN or LPN nursing license in good standing with the Virginia Department of Health Professions
- Wound care certification — or significant hands-on wound management experience
- Comfort collaborating across disciplines: nursing, MDS, physicians, and families
- Current CPR Healthcare Provider certification
- The ability to communicate complex clinical findings clearly to patients, families, and teammates
