Position Summary
This role is designed for you as a board-certified primary care physician (Internal Medicine, Family Medicine, or Emergency Medicine). You will offer medical expertise to help develop clinical guidelines for primary care doctors to use when they are managing virtual care patients who may require hospital-based care. You also will perform quality assurance of care delivered to virtual care patients who were referred for hospitalization.

- Key Admitting Privileges Required: Active hospital privileges in at least one hospital on this list: Albany Medical Health System or St. Peter's Health Partners
- Role Type: 1099 Independent Contractor
- Compensation Structure: Hybrid Framework
- Phase 1 (Clinical Guideline Project): A payment of $1,000.00 for the creation of a virtual care to hospital admission care-transition guideline and checklist. Expected time commitment for this project is 5 hours. Full project details to be shared and agreed upon prior to signing the contract.
- Phase 2 (Care Transitions & QA): Variable $200.00 per hour (pro rated for actual time spent) for all active services related to your handling of patient virtual-to-hospital admissions (including work requested to help with transition to hospital or quality assurance time spent reviewing hospital-based care of such patients). This phase 2 work to be performed strictly on an as-needed and as-available basis.
- Time Commitment: Variable / As-Needed.
Core Responsibilities1. Clinical Guidelines Project (Fixed $1,000 Fee)
This project involves creating a clinical guideline and checklist for primary care physicians to use when they are considering referring a virtual care patient for hospital care
- Protocol Localization: Audit, refine, and customize localized care-transition guidelines to align with local regional resources, hospital transfer center capabilities, and state telemedicine regulations. Full project details to be shared prior to signing the contract.
2. Care Transition & Admitting Coordination (Variable $200/Hour, As-Needed)
- Acute Escalation Management: In the event a regional virtual care patient requires inpatient care, we may request you on an as-needed basis to help manage the transition. If you are available and able to do this work:
- Handoff Execution: Review the patient's virtual clinical care report, coordinate the transfer logistics with the receiving facility, and execute a warm handoff to the inpatient medical staff using your local facility footprint.
3. Quality Assurance & Utilization Review (Variable $200/Hour, As-Needed)
- Post-Admission Case Review: Following a regional patient admission, you may be offered the opportunity to conduct objective chart and utilization reviews to evaluate the appropriateness of the escalation, care-delivery timelines, and medical necessity.
- QA Reporting: Identify operational gaps or clinical trends during one or more admissions to help refine the virtual clinic's regional triage algorithms.
4. Strategic Projects (Variable $200/Hour, As-Needed)
- Provide ad-hoc clinical consulting on localized healthcare regulations, state medical board nuances, and care-transition initiatives as requested by Health Tap Medical NY, P.C.
Position Requirements & Qualifications
- Education & Licensure: MD or DO from an accredited institution. Active, unrestricted medical license in New York with zero history of disciplinary action.
- Board Certification: Board Certified in Internal Medicine, Family Medicine, or Emergency Medicine.
- Mandatory Hospital Affiliation:CRITICAL REQUIREMENT: Applicants must currently hold active, unrestricted admitting privileges in good standing at: Albany Medical Health System or St. Peter's Health Partners.
- No exclusions: Not on the OIG, SAM, FDA Debarment, OFAC, or other regulatory exclusions/debarment lists.
- No Medicare opt-out: Not opted out of Medicare.
- Bylaw Compliance: Must independently verify that participating in an external 1099 care-transition and consulting agreement does not violate your current hospital's medical staff bylaws.
- Documentation Rigor: Must be willing to maintain and submit detailed, contemporaneous, detailed time logs for all consulting, care coordination, and QA services rendered outside baseline project hours.
Understanding Our Partnership
- Fair Market Value (FMV): Compensation is strictly tied to professional labor. All active patient management and care coordination is paid at the identical $200.00 hourly rate, which has been independently validated to reflect Fair Market Value for primary care administrative and consulting services in this region.
- Anti-Kickback & Stark Law Compliance: Compensation is paid solely for actual, documented professional labor, time, and deliverables. Payouts are entirely independent of, and decoupled from, the volume or value of any patient referrals or actual hospital admissions.
- Clinical Autonomy: The independent contractor retains absolute, unfettered clinical autonomy. The content of the clinical guidelines, the decision to admit or not admit a patient, and the content of QA review rests solely upon the physician’s independent medical judgment.
