Continuity Care Home Nurses, Inc. is seeking an experienced Registered Nurse Case Manager with a minimum of three years of experience to join our North Hollywood office team.
This is a full-time office-based position with hours of 10:00 AM to 7:00 PM, Monday through Friday. Candidates must have prior private duty home care experience and be willing to perform occasional home visits or patient shifts during emergencies.
Responsibilities
- Manage and coordinate private duty patient cases
- Conduct patient admissions and assessments as needed
- Coordinate care with physicians, case managers, insurance adjusters, and family members
- Schedule and monitor recertification visits
- Review clinical documentation for accuracy and compliance
- Follow up on laboratory results and physician orders
- Supervise field clinicians and caregivers
- Assist with staffing and patient care coordination
- Provide clinical support to office and field staff
- Participate in emergency patient coverage when necessary
Qualifications
- Current California RN License
- Three years of experience
- Private Duty Home Care experience required
- Strong case management experience preferred
- Knowledge of Title 22 regulations preferred
- Excellent organizational and communication skills
- Ability to manage multiple cases simultaneously
- Strong clinical documentation skills
- Detail-oriented with exceptional follow-through
- Proficient in time management and prioritization
