Job Summary
We are seeking a dedicated and energetic Physical Therapist to join our dynamic healthcare team. Responsibilities
- Conduct comprehensive patient evaluations to determine functional limitations and develop tailored care plans that align with medical goals.
- Administer evidence-based physiotherapy treatments such as trigger point therapy, manual therapy, and therapeutic exercises to facilitate recovery.
- Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless transitions between inpatient, outpatient, home health, or community care settings.
- Monitor patient progress regularly, adjusting care plans as needed to optimize results and meet individual needs.
- Document all patient interactions accurately within electronic medical record (EMR) systems while maintaining compliance with healthcare regulations.
- Educate patients and their families on injury prevention, home exercise programs, and ongoing care strategies to support long-term health.
- Stay current with advancements in physiotherapy practices, including specialized areas like orthopedics and geriatrics, to provide the highest quality care.
Skills
- Strong knowledge of physiology and human anatomy essential for accurate assessment and effective treatment planning.
- Proficiency in medical terminology relevant to physical therapy practices.
- Experience with discharge planning across various healthcare environments such as hospitals, clinics, outpatient facilities, and home & community care.
- Skilled in developing comprehensive care plans tailored to pediatric, geriatric, orthopedic, neurological, or general patient populations.
- Familiarity with EMR systems for documentation and patient management.
- Hands-on experience with physiotherapy techniques including trigger point therapy and other manual therapies.
- Excellent communication skills to educate patients effectively on treatment procedures and self-care strategies.
