Westminster Canterbury Richmond at Home is looking for a Speech-Language Pathologist for Home Health and Hospice to provide speech/language pathology and audiology services and ongoing assessment and evaluation of functioning for skilled patients. The Speech-Language Pathologist provides education and consultation with the patient/family and other organizational personnel. Accurately document evaluation and treatments provided.

The hiring range for this position is $50.45 to $67.26. Actual compensation depends on experience, skills, and internal equity.
MAJOR ACCOUNTABILITIES/ESSENTIAL JOB DUTIES:

Provide services in accordance with accepted standards of practice by or under the direction of a qualified Speech-Language Pathologist (SLP) or Audiologist.

Provide initial and ongoing assessments to identify the client’s level of functioning and ability to communicate, including OASIS assessments at appropriate time points
-
Develop the plan of care in consultation with the physician and other care team members, and revising it as appropriate
-
Prepare clinical and progress notes
-
Consult and coordinate services with other disciplines
-
Evaluate effectiveness and outcomes of care
-
Plan for discharge
-
Participate in in-service programs
-
Comply with agency policies and procedures relative to infection control and safety
REQUIRED KNOWLEDGE, SKILLS AND ABILITY:

Current and unrestricted Speech-Language Pathologist License in the Commonwealth of Virginia
- Current CPR certificate for healthcare provider
-
Must possess good verbal and written communication skills
-
Must be able to meet deadlines and work under limited supervision

Valid Driver’s License in the State of Virginia with a minimum age 21 to drive for the organization

Motor Vehicle Driving Record satisfactory to the Agency and its insurance providers
