Transformation Health, LLC is seeking an experienced, highly motivated, and results-oriented Program Director to lead our Grant Funded Community Supports Partnership Program serving youth, families, and schools throughout Baltimore City.
This is not a desk only leadership role. We are looking for a hands-on leader who thrives in fast-paced environments, values accountability, supports team development, and is passionate about improving outcomes for children and families.
The ideal candidate is a strong clinician, an effective supervisor, and a collaborative team player who leads by example. This position combines clinical leadership, program management, staff supervision, quality assurance, and community engagement.
The Program Director will maintain a regular presence within our OMHC program and periodically rotate among partner school locations to support staff, strengthen partnerships, and ensure high-quality service delivery.

Primary Responsibilities
Clinical Leadership
- Provide clinical oversight and consultation to licensed and non-licensed staff.
- Ensure fidelity to evidence-based and trauma-informed practices.
- Support treatment planning, case reviews, and clinical decision-making.
- Monitor quality of care and documentation standards.
- Promote Measurement-Based Care and outcome-focused treatment approaches.
Program Management
- Direct day-to-day operations of the Community Supports Partnership Program
- Oversee implementation of Tier 1, Tier 2, and Tier 3 behavioral health services.
- Ensure achievement of program goals, outcomes, and contractual deliverables.
- Monitor staffing, productivity, budgets, and program performance.
- Collaborate with school administrators, community organizations, and referral partners.
Staff Supervision & Development
- Provide administrative and clinical supervision to OMHC and Community Support program staff.
- Conduct regular team meetings, performance evaluations, and coaching sessions.
- Mentor clinicians pursuing advanced licensure and professional growth.
- Foster a culture of teamwork, accountability, and excellence.
School-Based & OMHC Leadership
- Maintain a regular on-site presence within Transformation Health's OMHC program.
- Conduct periodic site visits to participating Baltimore City schools.
- Support school-based staff and ensure quality service delivery.
- Build and maintain positive relationships with school personnel and community stakeholders.
Compliance & Quality Improvement
- Ensure compliance with CHRC, BHSB, Medicaid, OMHC, HIPAA, and organizational requirements.
- Oversee reporting, audits, quality assurance activities, and program documentation.
- Utilize data to support continuous quality improvement and informed decision-making.
Qualifications
Required
- Active Maryland independent clinical license:
- LCSW-C
- LCPC
- Minimum three (3) years of leadership, supervisory, or program management experience.
- Experience working with children, adolescents, and families.
- Experience in school-based, community-based, or outpatient behavioral health settings.
- Strong organizational, communication, and leadership skills.
- Reliable transportation for travel between school and clinic locations.
- Preferred-Maryland Board-Approved Clinical Supervisor designation.
- Experience supervising clinicians pursuing advanced licensure.
- Experience with OMHC operations and Medicaid-funded behavioral health services.
- Experience managing grant-funded programs.
- Knowledge of evidence-based behavioral health interventions.
