Position Summary
The Enrollment Specialist is responsible for driving patient participation in PRINE Health's Kidney Health Initiatives and related care management programs, with a primary focus on educating patients and enrolling them into Chronic Care Management (CCM) and other applicable programs. This role requires someone who can speak knowledgeably and persuasively about program benefits, build patient trust quickly by phone and in person, and manage a high-volume enrollment pipeline independently with minimal oversight. - Maintain and work an active pipeline of eligible/prospective patients, prioritizing outreach based on clinical urgency and enrollment deadlines

Program Support (CCM)
- Support light CCM care coordination tasks as needed (e.g., care plan check-ins, scheduling coordination, documentation of monthly time logs)

- Ensure enrolled patients' records reflect accurate program status and required documentation for compliance/billing purposes
Independent Case Management
- Manage own daily/weekly outreach schedule and enrollment targets without close supervision
- Identify patterns in declines or drop-off and adjust outreach approach accordingly
- Maintain organized, up-to-date records for every patient contact and enrollment status
- Report enrollment metrics and program progress to management on a regular cadence
