Duties
- Administer medications and treatments accurately following physician orders, including injections, IV infusions, and tube feedings.
- Conduct comprehensive patient assessments, vital signs monitoring, and physical examinations to evaluate health status.
- Develop and implement individualized care plans tailored to each resident’s needs, ensuring compliance with healthcare standards such as HIPAA and infection control protocols.
- Support residents with activities of daily living, including wound, tracheostomy or catheter care.
- Collaborate with interdisciplinary teams on discharge planning, case management, PT/OT/ST services, and post-acute care to optimize resident outcomes.
- Carrying out additional duties as delegated by the Director of Nursing or ordered by a licensed physician.
- CPR/First aid certified.
- Emergency response.

- Demonstrate correct and safe techniques in the use of equipment according to specific product information.
- Article 9 and Prevention & Support certification; Ability to utilize approved intervention and assistance techniques necessary to meet the needs of assigned individuals with developmental disabilities.
Experience
- Valid licensure as a Licensed Practical Nurse (LPN) with current state certification.
- Prior 1-2 years' experience working in long-term care facilities such as group homes, nursing homes or assisted living communities is preferred.
- Knowledge of infection control practices, IV infusion techniques, tracheostomy care, and wound management.
- Strong understanding of medical terminology, medication administration protocols, and patient assessment techniques.
- Ability to work effectively with individuals with disabilities or developmental challenges while maintaining compassion and professionalism.
WORKING CONDITIONS
- Work flexible shifts, varied days and times. This may include mornings, evenings, and weekends, depending on the needs of the individuals and the Company. All information must be secure and safe.
- Communications must be clearly and promptly provided to appropriate persons.
- All interactions and service delivery must promote customer relations.
