As a Home Health Aide, you will work closely with clients, families, and healthcare professionals to deliver high-quality care that promotes independence and comfort.
Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
- Monitor and record patient vital signs such as blood pressure, pulse, and respiration rates using appropriate medical equipment
- Support medication administration following prescribed guidelines and document medical records accurately in Electronic Health Record (EHR) systems
- Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques and behavioral management
- Perform catheter care and assist with hospice or end-of-life care needs with sensitivity and professionalism
- Observe and report changes in patient condition, behavior, or safety concerns promptly to supervising healthcare staff
- Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry, cleaning, and meal preparation in accordance with home & community care standards
Qualifications
- Proven experience working as a Home Health Aide or in assisted living/nursing home settings preferred
- Strong knowledge of medical terminology, patient care procedures, and home healthcare regulations
- Ability to assist with ADLs and provide compassionate dementia and Alzheimer's care
- Certification in CPR and Basic Life Support (BLS) required; First Aid certification is a plus
- Experience working with individuals with developmental disabilities or geriatrics highly valued
- Familiarity with EMR/EHR systems for medical documentation and record keeping
- Ability to lift heavy objects safely using equipment such as Hoyer lifts when necessary
- Excellent communication skills to collaborate effectively with patients, families, and healthcare teams
