About the Role
We are seeking a compassionate and motivated Licensed Master Social Worker (LMSW) to join our growing hospice team. The LMSW will provide psychosocial support, counseling, care coordination, and resource management for hospice patients and families while collaborating closely with the interdisciplinary team to ensure high-quality end-of-life care.
This position plays a vital role in supporting patient dignity, emotional well-being, caregiver education, and compliance with Medicare hospice regulations and CHAP standards.
Responsibilities
- Complete psychosocial assessments and ongoing patient evaluations
- Provide emotional support, counseling, crisis intervention, and grief support to patients and families
- Assist with advance care planning and community resource coordination
- Participate in interdisciplinary team (IDT) meetings and care planning
- Maintain timely, accurate, and compliant documentation within the EMR
- Educate patients and caregivers regarding hospice philosophy and available services
- Assist with discharge planning, placement coordination, and community referrals when appropriate
- Support compliance with Medicare Conditions of Participation, CHAP standards, HIPAA regulations, and agency policies
- Promote patient satisfaction, quality outcomes, and a 5-star hospice experience
Qualifications
- Current Arizona LMSW license in good standing
- Master’s degree in Social Work (MSW) from an accredited program
- Hospice, home health, palliative care, or healthcare experience preferred
- Strong communication, organizational, and documentation skills
- Ability to work independently and collaboratively within an interdisciplinary team
- Valid driver’s license and reliable transportation
Preferred Skills
- Knowledge of hospice eligibility and Medicare regulations
- Experience with EMR systems
- Compassionate bedside manner and strong family communication skills
- Bilingual English/Spanish preferred but not required

Pay: $35.72 - $43.02 per hour
