Job Responsibilities
- Coordinates home care services with clients and their families
- Reviews clinical components of care provided
- Ensures home care aides are supervised post-orientation in the home and that skills of the aide match the care needs of the client
- Visits referrals for home care to evaluate new clients' needs and start a plan of care
- Elevates any concerns in the home to the Home Care Director
- Schedules ongoing monthly visits with clients and communicates any changes to the care team
- Submits documentation in a timely manner
Key Qualifications
- RN degree
- One year experience in a health care setting, home care preferred.
- Solid understanding of home care and field staff disciplines
- Staffing flexibility
- Solid technology skills
- Demonstrated ability to build strong relationships
- High impact communication skills; both written and verbal
- Strategic thinker with strong attention to detail
- Self starter, able to prioritize, meet tight deadlines and manage time effectively
- Demonstrated ability to problem solve and implement solutions collaboratively
- Strong organizational skills
