Kline Galland Home is seeking an experienced and compassionate Director of Social Services to join our leadership team on a full-time basis. This key leadership role is responsible for overseeing social services programs, resident advocacy, discharge planning, and care coordination for our long-term care facility.
We are looking for a dedicated professional with proven experience in long-term care who is committed to enhancing residents' quality of life, supporting families, and ensuring exceptional social services throughout the continuum of care.
Key Responsibilities
Direct and oversee all social services functions within the facility.
Advocate for residents' rights, dignity, and overall well-being.
Lead discharge planning and care transition processes.
Coordinate with interdisciplinary teams to ensure comprehensive resident care.
Conduct psychosocial assessments and develop individualized service plans.
Provide guidance, support, and counseling to residents and families.
Ensure compliance with federal, state, and facility regulations.
Maintain accurate documentation and resident records.
Participate in care plan meetings, family conferences, and quality improvement initiatives.
Serve as a liaison between residents, families, staff, and community resources.

Qualifications
Required: Previous experience as a Director of Social Services in a Long-Term Care setting.
Strong understanding of long-term care regulations, resident rights, and discharge planning.
Excellent leadership, communication, and interpersonal skills.
Ability to work collaboratively with residents, families, physicians, and interdisciplinary team members.
Passion for resident-centered care and advocacy.
