Job Summary
We are seeking a passionate and dedicated Community Health Worker to join our team! In this vital role, you will serve as a bridge between community members and healthcare services, empowering individuals to achieve better health outcomes. Your enthusiasm and commitment will help foster trust, provide education, and support community members in navigating complex healthcare systems. Responsibilities
- Engage with community members to assess their health needs and provide culturally sensitive support
- Educate individuals on health topics such as nutrition, addiction counseling, and preventive care
- Assist clients with understanding medical terminology and navigating Medicare or other insurance programs
- Conduct data collection related to patient interactions, health outcomes, and service needs
- Use motivational interviewing techniques to encourage positive behavioral changes
- Collaborate with healthcare providers to coordinate care for individuals with disabilities or chronic conditions
- Deliver public speaking presentations on health topics at community events or workshops
Skills
- Strong knowledge of nursing principles and public health practices
- Experience working with individuals with disabilities or in social work environments
- Excellent teaching and public speaking abilities to engage diverse audiences
- Proficiency in data collection methods and understanding of medical terminology
- Familiarity with Medicare policies and nutrition education strategies
- Skills in motivational interviewing techniques to promote behavioral change
- Ability to provide addiction counseling support when needed
