About the Role:
As a Registered Nurse with Concord Care of Hartford, you'll have the opportunity to work in a dynamic and supportive environment that prioritizes staff appreciation and job satisfaction. You'll work closely with our interdisciplinary team to deliver high-quality patient care, develop individualized care plans, and provide emotional support to our residents and their families.
Responsibilities:
- Provide direct patient care, assessing patients' physical and emotional needs, and developing effective care plans
- Collaborate with other healthcare professionals to ensure seamless care transitions and coordinate patient care
- Educate patients, families, and caregivers on disease management, medications, and treatment plans
- Maintain accurate and detailed records of patient care, including assessments, treatments, and medications
- Participate in quality improvement initiatives, identifying areas for improvement and implementing changes to enhance patient care
- Demonstrate compassion, empathy, and respect for patients, families, and colleagues
Shift Requirements:
We currently have the following shifts available:
- 1 full-time shift from 6:00a-6:30p
- 1 part-time shift from 6:00a-6:30p
- 1 full-time shift from 6:00p-6:30a (night shift)
