Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patient conditions, accurately documenting vital signs and changes in health status using Electronic Medical Record (EMR) systems
- Support medication administration and ensure adherence to prescribed treatment plans in compliance with home healthcare regulations
- Provide specialized care such as Alzheimer's, dementia, memory care, hospice, and end-of-life support with sensitivity and professionalism
- Safely transfer patients using patient transfer methods and equipment like Hoyer lifts or other assistive devices
- Maintain patient confidentiality by adhering to HIPAA guidelines and safeguarding medical records and personal information
- Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment
Requirements
- Proven experience working with individuals with disabilities or in assisted living environments is highly preferred
- Knowledge of medical terminology, catheter care, vital signs monitoring, and basic life support (BLS) or CPR certification
- Prior experience in geriatrics, long-term care, or home healthcare agencies is advantageous
- Ability to lift heavy objects safely and perform physically demanding tasks as needed for patient transfer or mobility assistance
- Familiarity with EMR/EHR systems for accurate medical documentation and record keeping
- Strong communication skills to effectively interact with patients, families, and healthcare teams
- Valid certification in first aid and CPR; additional certifications related to hospice or dementia care are a plus
- Must comply with all home health care regulations and maintain high standards of patient care and safety
